I have a couple of hoarding anecdotes but I don't want to talk about them on the internet because they involve people I know. I suppose the reality show aspect could be an intervention and beneficial (I'm sure the producers would like us to think that). Your example of a couple is more sympathetic than what I imagined, which is the lone recluse or cat lady suddenly having a spotlight shined in her burrow. That seems like it could be more damaging than just being left alone. I'm not saying I wouldn't enjoy the show--I spoiled a party conversation recently with a depressing-but-funny hoarder story (involving someone I did not personally know).
